Caregiving is the heart of Caregivers by WholeCare continuum of care. Our caregivers are an extra set of hands (and eyes and ears) available to handle the many activities of daily living – working in tandem with family and/or facility staff to provide the best quality results.
Our caregivers are employees, not independent contractors, and in addition to providing general care, companionship, and respite, are trained to deal with a variety of diagnosis including:
- Post Surgery Care at home
- Post Stroke Care
- Cancer Treatment
Care is flexible and integrated to fit the needs of all parties and provide the person-centered approach that improves quality of life for all involved.
Types of Caregiving
Personalized Care Plan
In order to provide appropriate and quality services for the Client, a Client Care Coordinator develops a personalized care plan (the “Care Plan”) for you or your loved one. This Care Plan begins with an initial written assessment and recommendations for meeting the Client’s non-medical,
individual needs and concerns. This Care Plan helps to ensure and support the continuity of care for the Client. Care plans are always flexible and can be amended as needed.
The Care Plan is based on:
- a staff assessment that includes a comprehensive history of your medical, mental and emotional and spiritual needs and wishes.
- a brief home safety overview.
